Substitute Freddie Ladapo gave the visitors the lead just after the hour mark, but Jordan Clark immediately equalised to grab his third goal in four games.
However, it was another substitute who secured all three points for Paul Warne's side, with Ben Wiles glancing a header into the far corner in the 91st minute.
